Nursing Home Care

For patients without local family and friends to assist with in-home care, Hospice provides care in local nursing homes. Whether patients become part of Hospice before or after entering a nursing home, they receive the same excellent care they would in their own homes.

Recognizing nursing homes expertise in patient care, Hospice enhances that care for patients living at the end of life. These combined strengths of nursing home staff and of Hospice staff create quality, compassionate care for nursing home patients and their families. Hospice supplements nursing home care by offering these services:

  • A registered nurse serving as Hospice's nursing home coordinator
  • Specially-trained nurses implementing home care, pain management, and symptom control
  • Personal care specialists assisting patients with basic hygiene
  • Social workers providing psychological and emotional support to patient & family
  • Spiritual counselors consulting with patient and family members about spiritual concerns in conjunction with the patient's own minister or priest
  • Trained volunteers offering companionship and comfort
  • Bereavement counseling providing grief and bereavement support during and after the patient's death
